Confidentiality and consent


All your answers will be made anonymous and only used for research purposes. No identifiable information will be given to anyone else. 


If you agree, we will occasionally send you questionnaires about your health. Also with your permission, the researchers will look at your NHS health records (your prescriptions, diagnoses, and GP/hospital visits).


Once you have completed the Health Questionnaire you will then be eligible to take part in health studies with our researchers. If you agree we will also contact you about health research studies that you might be interested in.


It is completely up to you whether or not you decide to take part in these additional health studies or whether or not you fill in any more Health Questionnaires.


You can withdraw your consent from the study at any time.
